**We are seeking a Community Supervisor for our newly established MGH2Home Program - FT Contract**
**What will be your role?**
This is a unique leadership position supporting East Toronto Health Partners’ (ETHP) strategy to improve client transitions from hospital-to-home through the newly established MGH2Home program. This short-term transitional program involves linking together Michael Garron Hospital (MGH), Spectrum Health Care, VHA Home HealthCare and WoodGreen to support earlier discharges of adults with higher needs to reintegrate into their home and community. This multi-disciplinary team will work as “one team”, along with the client and designated family, to address their complex health and social needs in the development of a single “one team” care plan.
The Community Supervisor will take the lead in identifying clients, developing and validating comprehensive care plans and ensuring smooth transitions from the hospital. In addition, the Community Supervisor will provide oversight to ensure coordinated scheduling across organizations, coach an integrated care approach within the team across the partners, and ensure ongoing communication with primary care, home and community care support services and other community services. This leadership position will provide direction and operational oversight to both regulated and unregulated health professionals and will have shared supervisory accountability for nursing, personal support, OT, OTA, and homemaking roles.

**Required Qualifications**:
- Undergraduate Degree in Nursing/Science in Nursing and current certificate of competency (licensure) from the College of Nurses of Ontario required.
- 5-years progressive experience in nursing of which 2-years include experience in a supervisory or advanced practice role.
- Good understanding of the health care services and delivery processes in the community and home care sector.
- Experience working within an interprofessional or integrated care approach is essential.
- Knowledge and experience with gerontology
- Demonstrated leadership skills with strong organizational and self-management skills, and with demonstrated ability to achieve personal, professional, and organizational performance targets.
- Demonstrated knowledge and/or proficient skills in the use of information technology for electronic clinical documentation and clinical decision support. Knowledge and experience in use of standardized assessment tools in care plan development an asset
