_**RAPS is Seeking a Part Time Caregiver for an Individual with Dementia and Diabetes (S14)**_

- **(Approximately 25-30 hours per week:
6-8 hour shifts)**_
- _Maintain a safe and healthy environment_
- Duties include but are not limited to:
_
- _Prompting and actively participating in the daily routine_
- _Personal care assistance including bathing, dressing, changing incontinence products, range of motion exercises, medication administration, etc._
- _Light housekeeping and laundry_
- _Healthy home cooked meal preparation_
- _Monitoring food intake and blood glucose levels_
- _Support on outings if necessary_
- _Supervising and acting proactively by redirecting, cueing, and guiding the individual_
- _Providing companionship by engaging and interacting during daily activities_
- **Rate of Pay**:
