As a Social Work Assistant with VCH you will:
Work under the general operational supervision of the Patient Services Manager, clinical supervision from the Practice Coordinator and receive work direction from Social Workers in a team based environment.
Provide social work support services during the admission and discharge of patients/clients.
Perform social work duties including:
Interviewing clients/families/facilities for information,
Assisting clients to approach external resources,
Providing practical support to clients regarding benefits, housing, transportation, community resources, finances,

QUALIFICATIONS:
Education &
Experience
Completion of a recognized Social Service Worker or Social Service Diploma Program or an equivalent combination of education, training and experience.
Knowledge, Skills &
Abilities
Ability to communicate effectively both verbally and in writing.
Ability to deal with others effectively.
Physical ability to carry out the duties of the position.
Ability to priorize and organize work.
Ability to operate related equipment.
