**1 ‑ Temporary Full Time ‑ Social Worker BSW**

Reporting to the Reporting to the Manager ‑ Ambulatory Mental Health and Supportive Care Services, the successful

Applicants must have the following qualifications:
- Honors Bachelor of Social Work degree
- Current registration with the Ontario College of Certified Social Workers
- Minimum of 2 years experience in an acute care environment
In addition, applicants must have skill and experience in the following elements of practice:
- Relevant clinical experience with Chronic Disease and multiple/complex medical issues
- Strong assessment and counseling skills
- Excellent written and verbal communication skills
- Displayed abilities in Critical thinking, problem solving and conflict resolution
- Proven abilities to work well under stress and in a constantly changing environment
- Experience in teaching acutely ill patients and their families
- Displayed awareness of appropriate community resources
- Experience practicing as an active participant on a multidisciplinary team
- Experience working with people at end of life and in the setting of bereavement
- Excellent knowledge of community resources is essential
Salary Range:
$35.420 to 45.130
