**Haldimand Family Health Team Employment Opportunity**
**POSITION:
Social Worker (MSW) - Full Time (1.0 FTE)**
An exciting opportunity is now available for a Social Worker to work as a member of the
Haldimand Family Health Team, serving approximately 27,000 patients in Caledonia, Hagersville and Dunnville areas.
The Social Worker provides assessment, ongoing treatment and management of the biopsychosocial aspects of health and well-being of our clients, system navigation, group and community presentations and participates in the development, implementation, monitoring and evaluation of programs to meet the needs of the patients and community. Working in collaboration with an inter-professional healthcare team, the Social Worker assists health promotion, disease prevention, and chronic disease management across a patient’s lifespan.
This position will require working at more than one site and an Ontario drivers licence and a reliable vehicle is required.
**QUALIFICATIONS**:
- Masters Degree in Social Work from a recognized university
- Current and in good standing registration with the Ontario College of Social Workers and Social Service Workers
- Current liability insurance
- 3-5 years experience in a primary care setting is preferred
- Experience providing counselling / psychotherapy to adults, teens, couples and families
- A thorough knowledge of and proficiency in current assessment and psychosocial counselling techniques, including CBT, Brief Solution Focused and Motivational Interviewing essential
- Proficiency in crisis managements skills and suicide risk assessments
- Knowledge of current mental health and privacy legislation
- Case management experience preferred
- Understanding of social determinants of health and their impact on health and well
- being

**Salary**:
$70,948.00 - $74,148.00 - dependant on relevant experience
