Under the direction of the Professional Practice Leader, and in consultation with the Program Manager, the Social Worker is responsible for activities and intervention pertaining to patient care planning and delivery. Accountability is established through ongoing supervision and consultation, clinical recording, audit and performance appraisals. The Social Worker will adhere to worker responsibilities as set out in the Occupational Health &
Safety Act, hospital safety policies, and dept/unit established procedures at all times.
Within this role the employee is accountable for contributing to the delivery of the Kingston Health Sciences Centre strategy. As an employee, one must demonstrate an awareness of and be responsible for actively promoting and supporting patient and family centered engagement and care in all we do
**PRINCIPLE DUTIES &
RESPONSIBILITIES**:
- Formulates psychosocial assessments on the basis of interview with patient, family and significant others.
- Plans, organizes and establishes patient care priorities based upon identified needs and modifies these priorities as needed.
- Provides therapeutic interventions to patients and their families (i.E. Crisis, adjustment, bereavement, psychotherapy, resource counselling).
- Develops and evaluates documentation of assessments, plan and intervention.
- Serves as resource and social work consultant to patients, families, colleagues and health care professionals for enhancement of patient care.
- Formulates/coordinates discharge plans for all assigned patients.
- Initiates referrals to hospital and community based programs as identified through clinical/therapeutic assessment through the discharge plan.
- Provides patient care and family education as required.
- Serves as an advocate for patients and their families.
- Serves as a liaison and resource to community agencies and services promoting continuity of care of patients and families from hospital to home/facility setting.
- Communicates and consults with other health care professionals and participates as a member of the multidisciplinary health care team regarding client psychosocial issues and/or discharge plans.
- Participates in the orientation of new personnel and facilitates educational opportunities for health care colleagues.
- Participates in the social work student program in the role of field instructor. Directs, supervises and evaluates activities and care provided by students.
- Maintains competence and currency in social work and health care issues through in-service and continuing education.
- Initiates and/or participates in research activity.
- Identifies policy gaps related to patient care and initiates appropriate action.
- Participates in Social Work Professional Practice meetings and activities.
- Interprets hospital policies to patients, families and visitors.
- Participates in the hospital and social work quality improvement program.
**QUALIFICATIONS**:
- Masters Degree in Social Work required.
- Eligible for membership in the Ontario College of Social Workers and Social Service Workers (OSWSSW).
- A minimum of two years health related experience preferred.
- Demonstrated assessment and therapy skills with expertise in individual, couple, family and group work required.
- Knowledge of relevant legislation and community resources as well as an understanding of the discharge planning process required.
- Demonstrated ability to function independently and as part of a multidisciplinary team.
- Teaching experience within academic and/or clinical settings preferred.
- Superior communication skills required, oral and written.
- Proven ability to attend work regularly.
- Satisfactory criminal reference check and vulnerable sector search required.
**PHYSICAL REQUIREMENTS**:
The incumbent will be required to meet the physical demands as described for the area of work.
